My doctor recently used a different laboratory for medical tests. This lab is not covered by my health insurance. I was never even told about the change and now I have a $200 bill to pay. Aren't patients supposed to be notified of these kind of changes?

We mistakenly thought we had 60 days to add our newborn son to our existing health insurance policy when we only had 30. Our insurer has denied him coverage, so we have filed a written appeal.
