Do both parents have to sign on a child life insurance policy as owners?

No, both parents don't have to sign for a life insurance policy on a child.

There is no requirement by life insurance companies for both parents of a child to consent to a life insurance policy taken out on their child.  Of course, it would be helpful if both parents were involved and agreed on the terms of the policy.
